To qualify for our dealer program and special pricing, you/your business must:

• Have and provide a copy of your state-issued Sales Tax and Use Permit or equivalent document.
• Engage in one of the following primary business functions: Sales/Installation of security/surveillance equipment, computer sales or related services, or sales/installation of electronic equipment.
• If you are a reseller in the state of Illinois, you must fill out and provide the Illinois Sales and Use Tax Certificate. Please email to Support@CCTVHotDeals.com or fax to 217-351-7825.

Important Information for Applicants:

• Please fax the completed form, along with a copy of your state-issued Sales Tax/Resale Permit, to the number indicated on the application.
• Please allow up to 10 business days for research and processing. Applicants will be notified via e-mail upon completion.
• Please complete the form in its entirety and provide the requested state-issued tax documents.
• Applications received with missing or incomplete information may be declined without notice.
